Consultation requests

The callback form asks for your name, phone number and permission to contact you. It also sends your selected interface language. These details are used to coordinate your request, not to subscribe you to marketing. Do not include medical information. Sending a request does not confirm an appointment.

Delivery and storage

Requests are sent through the configured email provider to the centre’s receiving mailbox. This form does not create a website database record or save the fields in browser storage. An email provider and authorised centre staff can access the message. Details remain in the open form after an error so you can retry. Database and email retention and deletion are managed by the centre; contact the centre to request access, correction or deletion.

Technical data and analytics

The server uses your network address temporarily to limit repeated submissions. After a successful request, it stores a keyed hash of that address to prevent another automatic consultation popup from the same IP. The raw address is not stored in the consultation database. Hosting services may retain technical access logs. The site uses Google Analytics and its cookies for visitor statistics. The consultation feature does not send names, phone numbers or field values to analytics. Browser settings can restrict cookies.

Other contact services

WhatsApp and map links open external services governed by their own privacy terms. A WhatsApp draft is not sent until you send it in WhatsApp. You can call instead if a map or messaging service is unavailable.

Newsletter

The Persian newsletter is a separate opt-in service. Its contact details, consent and verification process are described in the Persian privacy notice. Submitting a consultation request does not enrol you.
